Healthcare in Asheville for Retirees: Hospitals, Medicare, Care

You can get good healthcare here, and you'll have more choices than most people expect: four hospitals across three counties, a full-service VA hospital, and two more hospitals in the pipeline. The catch is access. The main hospital runs very full, it has a rocky federal safety record, and new patients can wait months for a primary care doctor. The couples who do well line up their doctors before the moving truck shows up.

Before real estate, I was a nutrition scientist: a PhD in nutritional sciences at UF, then a postdoc at Harvard. So I read hospital news more closely than most brokers do. I'm still not your doctor, and I won't tell you where to get care. What I can do is give you the dated facts, tell you how far a house is from each hospital on this page, and make sure Mission's story doesn't come as a surprise after you've moved.

The hospitals, in plain terms

Mission Hospital (Asheville). HCA Healthcare bought the six-hospital Mission Health system for $1.5 billion in 2019. Mission Hospital is the region's main trauma and referral hospital, so if something serious happens anywhere in the area, this is often where you end up. It's also very full: average occupancy in 2025 was 99.7%, against a 78% target (NC Health News, Sept 15, 2026).

AdventHealth Hendersonville (formerly Park Ridge). A faith-based nonprofit community hospital at 100 Hospital Drive in Hendersonville that has been adding clinics and outpatient space.

UNC Health Pardee (Hendersonville). Affiliated with UNC Health. Pardee reports 5-star CMS ratings and stroke awards on its own website. Check those on Medicare's Care Compare tool before relying on them.

Haywood Regional Medical Center (Clyde, near Waynesville). A Duke LifePoint hospital with 11 specialty clinics and urgent care in Hazelwood and Canton.

Charles George VA Medical Center (1100 Tunnel Road, Asheville). A full-service VA hospital: 24/7 ER, inpatient care, surgery, cancer care, cardiology including cardiac cath, long-term care and hospice. If one of you is a veteran, this is a real asset.

MAHEC (Mountain Area Health Education Center) is a teaching organization with primary care and specialty practices. It trained 218 residents and fellows as of April 2024.

Mission Hospital's history, stated plainly

People ask me about this a lot, usually starting with "I read something..." Here's the timeline, with dates:

  • 2019: HCA buys Mission Health. Afterward, nearly 100 physicians left and several clinics closed, and Pardee and AdventHealth expanded to fill the primary care gap (Hendersonville Lightning, 2021).
  • 2021: CMS issues an "Immediate Jeopardy" (IJ) finding after a patient was found dying on her room floor.
  • Late 2023 to mid-2024: Inspectors found 18 patients harmed in 2022-23, including four deaths, with problems in emergency and cancer services. IJ announced February 2024, lifted June 2024.
  • December 2023: then-Attorney General Josh Stein sues HCA, alleging it broke the 2019 purchase agreement by degrading emergency and cancer care.
  • August 2025: HCA settles a separate antitrust suit brought by western NC local governments.
  • October 2025: New IJ over patient misidentification, telemetry monitoring and transport safety. Mission's CEO disputed many of the complaints.
  • January 2026: Another IJ after a survey. In February, CMS rejected Mission's correction plan, citing "systemic and recurring patterns" of harm.
  • July 2026: A Business Court judge denied HCA's motions for summary judgment, so the AG's case goes to trial. The independent monitor under the sale contract reported "potential noncompliance."
  • August 4, 2026: CMS notified Mission that it is in substantial compliance and no further surveys related to the January 2026 survey are needed. Mission is keeping consultants on through the end of 2026.

That's where things stand as of late September 2026. This story has changed direction several times in five years, so check the news before you decide anything. What I'd do with it: know which hospital you'd prefer for planned care, ask your doctors where they have privileges, and ask where the specialists you need actually practice.

New hospitals coming (slowly)

  • AdventHealth Weaverville (north Buncombe) broke ground March 26, 2026. Phase 1 has 67 beds with an ER, ICU and imaging. NC Health News has reported a February 2028 target. AdventHealth hasn't announced a date.
  • Novant Health does not have an Asheville hospital today. In March 2026 it won state approval for 34 beds at a new hospital in Arden (south Buncombe), targeted for around 2030. That approval is under appeal.
  • Mission won approval for 95 more beds in the same March 2026 round, also under appeal. Reports disagree on whether they'd go on the Asheville campus or in southern Buncombe.

Appeals have delayed all of these. Treat any opening date as a hope.

Primary care: start before you move

There are no hard regional numbers on wait times, but local doctors and AdventHealth told WLOS in April 2024 that new-patient primary care appointments can be "several months out." A MAHEC physician said "every county in Western North Carolina is a primary care shortage area."

What couples who handle this well tend to do:

  • They start calling practices a few months before the move and ask whether the practice keeps a waitlist.
  • They ask each practice which hospital its doctors admit to.
  • They request copies of recent labs, imaging and medication lists from their Florida doctors before they leave.
  • If they have a Florida specialist they trust, many keep that doctor for a while (see the flights section below).

I was a scientist before real estate, so I like to see the numbers, and a simple spreadsheet helps here: each practice in a row, with the phone number, the date you called, whether they keep a waitlist and which hospital they admit to.

Medicare when you move from Florida

This is general information about how the system works. For your own plan choice, NC's free SHIIP counselors or a licensed Medicare agent are the people to call.

Medicare Advantage plans are local. An MA plan is tied to its service area, so moving from Florida to North Carolina generally opens a Special Enrollment Period to pick a new plan. Aggregator sites counted 49 to 67 Medicare Advantage plans for sale in Buncombe County for 2026, from carriers including Humana, UnitedHealthcare/AARP, Anthem BCBS, Aetna, HealthSpring, Wellcare and Devoted. Use Medicare.gov's Plan Finder as the official list, since the commercial sites don't always agree.

Medigap moves with you. A Medigap policy works nationally with Original Medicare. That flexibility matters to some people who want to keep seeing doctors in two states.

Switching later can be harder. Moving from Medicare Advantage to Original Medicare plus Medigap after your first year can mean medical underwriting unless you have a guaranteed-issue right. Ask SHIIP how that applies to you before you choose.

Check the network, every year. As of Sept 23, 2026, I found no reporting that Mission, AdventHealth, Pardee or Novant were dropping Medicare Advantage plans for 2027. That can change, and open enrollment runs October 15 to December 7. Before you pick a plan, confirm that your doctors and your preferred hospital are in it.

LGBTQ-affirming care

  • Western North Carolina Community Health Services (WNCCHS), 257 Biltmore Ave, Asheville. A federally qualified health center providing primary care "regardless of ability to pay," with a Transgender Health Program. Explore Asheville's LGBTQ+ guide lists it as affirming for trans and nonbinary patients (Nov 2025).
  • WNCAP (Western NC AIDS Project): HIV and hepatitis C care, PrEP and harm reduction.
  • Planned Parenthood Asheville Health Center lists gender-affirming hormone therapy.
  • Campaign for Southern Equality, headquartered in Asheville, runs the Trans in the South directory of trans-affirming providers, and published an advisory on adult care barriers in May 2026.
  • OutCare keeps a national directory of affirming providers, with a North Carolina list.
  • PFLAG Asheville maintains a list of LGBTQ+-affirming therapists.

A few honest notes. I could not confirm any Asheville hospital system as a current "Leader" in HRC's Healthcare Equality Index, so ask each practice about its nondiscrimination policy and staff training directly. Gender-affirming care is legal for adults in North Carolina. The NC State Health Plan (for state employees and teachers) excludes transition care, and that exclusion stands after a lawsuit was dismissed in June 2026. CSE has also warned about preemptive clinic closures and insurance exclusions, so confirm coverage before you rely on a provider.

Flying back to your Florida doctors

Some couples keep a Florida specialist for a year or two after the move. Asheville Regional Airport (AVL) makes that easier than you'd think. As of the airport's nonstop destination listings (fetched September 2026):

  • Allegiant: Fort Lauderdale (year-round), St. Pete-Clearwater (year-round), Orlando Sanford (year-round), Orlando International, Punta Gorda, Sarasota, West Palm Beach, Key West, and Destin in summer
  • American: Miami, daily for most of the year

There's no nonstop to Tampa International or Jacksonville. Allegiant flights are often less than daily, so plan appointments around the schedule. For anything else, Charlotte's airport is about 2 hours 13 minutes away without traffic.

The straight talk

  • The main hospital has a troubled record. Mission received federal Immediate Jeopardy findings in 2021, February 2024, October 2025 and January 2026, and the AG's lawsuit is heading to trial. It was found back in substantial compliance in August 2026. You deserve to know all of it.
  • Beds are tight. Mission ran at 99.7% average occupancy in 2025, and new capacity is years away and tied up in appeals.
  • Primary care is the bottleneck. If you move first and look for a doctor second, you may wait months.
  • Some specialty care means driving. Map your specific specialists before you choose a town. A home near Hendersonville puts you close to two hospitals. A home near Waynesville puts Haywood Regional nearby and Asheville farther away.
  • You'll vet affirming care yourself. Good clinics and directories exist, and I could not confirm any hospital system here as a current HRC Healthcare Equality Index "Leader."
  • Some Medicare choices are hard to undo. SHIIP's free counselors can walk you through the options before you pick a plan, especially if you're thinking about Medigap.

Questions people ask

Is Mission Hospital safe? That's a judgment I'm not going to make for you. Here are the dated facts: Mission had federal Immediate Jeopardy findings in 2021, 2024, October 2025 and January 2026, and CMS found it in substantial compliance on August 4, 2026. Read the latest reporting, talk to your doctors about where they admit patients, and decide what fits your needs.

Is Novant building a hospital in Asheville? Not in Asheville itself. Novant won approval in March 2026 for a 34-bed hospital in Arden, in south Buncombe, targeted for around 2030, and that approval is under appeal. Novant has no hospital here today.

How long does it take to get a primary care doctor? There's no official regional statistic. Local doctors told WLOS in 2024 that new-patient appointments can be "several months out," so many couples start calling practices a few months before the move.

Will my Florida Medicare Advantage plan work here? Medicare Advantage plans are tied to a service area, so under general Medicare rules you'll need a new plan when you move, and the move opens a Special Enrollment Period. Are there LGBTQ-affirming doctors in Asheville? Yes. WNCCHS has a Transgender Health Program, WNCAP offers HIV care and PrEP, and Campaign for Southern Equality's Trans in the South guide and the OutCare directory list affirming providers. Ask any new practice about its nondiscrimination policy and staff training.

Can we still fly to our doctors in Florida? Yes. AVL has nonstops to about ten Florida airports, mostly on Allegiant, plus American to Miami. Many run less than daily, so check the schedule before booking appointments.
